Suggestions for a successful visit:
Patient Forms
Forms require Adobe Reader click here to download for free ReferralsIf your insurance requires a referral, you are responsible for obtaining the referral and seeing it reaches our office before your appointment. We also ask that you have your physician send any documentation, test results, etc. that would help our physicians understand your problems and understand what they are to see you for on this visit. This will also eliminate the need for duplication of tests in order to help diagnose and treat your symptoms. Same Day AppointmentsPatients with acute/urgent conditions that need to be seen immediately will be "worked into the day’s schedule". We understand that you are sick and will do our best to have you in an exam room as quickly as the physician's schedule will allow. Please be aware there may be a wait as the patients with "scheduled appointments" need to be seen and we are seeing you without an appointment. InsuranceWe take most major insurance plans. Please check with your insurance company to see that our physicians are listed on your plan. You need to understand your benefits. Do you have a deductible that must be met? Do you have a specialist co-pay? Could the condition for which you are seeking treatment be considered pre-existing? Please note that we collect insurance co-pays and deductibles at the time of your visit.
