About Your New Patient Visit

We look forward to meeting you on your initial visit to our office. At this time, we'll help you get acquainted with our services and learn about your particular diagnostic and treatment needs. Our electronic medical record allows us to provide you with the highest quality care and allows for optimal communication with your other health care providers. In addition, we know that every patient is different and has specific needs, and we will strive to provide you with personal, individualized attention.

You can make a difference in the quality of care that you or your child receive. Remember to ask questions, keep an accurate medical history, understand how to administer all medications, understand what you or your child's test results mean, and what to expect if the physician recommends surgical treatment for your or your child.

Prepare for your new patient visit:

Bring your Health Insurance Card or Information
Bring a list of all medications you are taking
Bring a copy of any sleep, studies, X-RAYS or CT scans taken in the last year specific to the condition we are seeing you for (not just the reports - get the actual films on disk if possible)
Bring a copy of any medical record or test result specific to the condition we are seeing you for
Bring a family member to take notes/help with questions especially if there is a hearing loss or speech barrier or if the patient is coming from a nursing home
Bring written parental authorization if someone other than parent/legal guardian accompanies a child 0-18 years of age (see consent form below)
Bring your insurance referral (if one is required by your insurance plan)
Fax completed Advanced ENT and Allergy new patient forms (see below) in advance of your appointment
Understand your insurance benefits and come prepared to pay your insurance co-pay and/or deductible
Leave yourself plenty of time. Don't create more stress for yourself by scheduling something else right on the heels of your new patient appointment.

Suggestions for a successful visit:

Take down some quick notes on answers to questions, or other comments made by the doctor.
Don't be afraid to ask questions: Ask for the spelling of tests, medication, or other unfamiliar words. What is the name of my (child's) condition (also called a diagnosis)? What treatments are you recommending, and why? What are my (child's) treatment options? If this is a long-term problem, what can I expect in the future (the prognosis) for my (child's) health?
Ask the doctor if there is any reading material (books, pamphlets, etc.) available that may help answer any questions you might have. Some questions about tests, procedures, and medications may be thoroughly answered through pamphlets, and the doctor can offer some highlights during your appointment.
Repeat treatment directions back to the doctor or his clinical team to make sure you understood them

Referrals

If your insurance requires a referral, you are responsible for obtaining the referral and seeing it reaches our office before your appointment. We also ask that you have your physician send any documentation, test results, etc. that would help our physicians understand your problems and understand what they are to see you for on this visit. This will also eliminate the need for duplication of tests in order to help diagnose and treat your symptoms.

Same Day Appointments

Patients with acute/urgent conditions that need to be seen immediately will be "worked into the day’s schedule". We understand that you are sick and will do our best to have you in an exam room as quickly as the physician's schedule will allow. Please be aware there may be a wait as the patients with "scheduled appointments" need to be seen and we are seeing you without an appointment.

Insurance

We take most major insurance plans. Please check with your insurance company to see that our physicians are listed on your plan. You need to understand your benefits. Do you have a deductible that must be met? Do you have a specialist co-pay? Could the condition for which you are seeking treatment be considered pre-existing? Please note that we collect insurance co-pays and deductibles at the time of your visit.

We invite you to log in to your online account or create a new online account. Take advantage of the online services offered by our practice with the assurance that all of your information is encrypted and stored securely.

In our patient portal you can: Update your address, phone, e-mail address, insurance and other patient information.
Change your password.
Add and update your credit card information.
View a history of your previous online visits.

After you create an online account, you will receive an e-mail from us that includes your user ID and a link to our patient portal. You may log in at any time to activate your account and begin using our online services.
