Adeline Dixon is a board-certified Physician Assistant from Jeffersonville, IN. She earned her bachelor’s degree in Human Biology with a concentration in Human Health and Disease from Indiana University Bloomington and her master’s degree in Physician Assistant Studies from Midwestern University. Adeline is nationally certified and licensed to practice in Indiana and Kentucky. In her free time, she enjoys traveling to new places and participating in various recreational sports leagues.

Carly grew up in Louisville and graduated with her bachelor's in nursing from Spalding University in 2013. She primarily worked in pediatrics until earning her master's degree at the University of Louisville and becoming a certified Family Nurse Practitioner in 2017. She has been with Advanced ENT and Allergy since 2022. In her free time, she enjoys spending time with her husband and two daughters.

Nicole Hardy, PA-C, MSPAS

Nicole Hardy, PA-C, has been with Advanced ENT and Allergy for over ten years, working with Dr. Keith Forwith’s team. While they specialize in thyroid and parathyroid disorders, they also manage a wide range of other ENT conditions. Nicole has a particular interest in the management and treatment of Hashimoto’s thyroiditis. She completed her undergraduate studies at Transylvania University and earned her PA degree from the University of Kentucky.

Amber is originally from Grayson, KY. She received her bachelor’s degree in Biomedical Sciences from Morehead State University. She received her master’s degree in Physician Assistant Studies from Sullivan University. She is a nationally certified Physician Assistant and is licensed to practice in Indiana and Kentucky. In her free time, she enjoys outdoor activities and traveling with her husband.

Mallory Swanson, DNP, FNP-C, APRN

Mallory is a certified Family Nurse Practitioner with AANP. She completed her doctorate in nursing at the University of Louisville in 2023. Prior to that, Mallory spent five years as a nurse at the bedside in the Neonatal ICU and Norton Children’s Hospital.

After growing up in the Indianapolis area, Mallory moved to Louisville for her undergraduate degree and has been here since. She was on the Division I rowing team at University of Louisville while she got her BSN. She is married to her husband, Zach, and together they have a yorkie named Kip.

Asia is originally from Bethany, CT. She received her B.A. from Colgate University in Molecular Biology where she graduated Magna Cum Laude. From there she moved with her military spouse to Washington State where she attended Pacific Lutheran University and graduated from their Entry Level Masters Program with her R.N. and a Masters of Science in Nursing in 2006. She is a Board Certified Family Nurse Practitioner and has been working in ENT for approximately 6 years, with the last year as part of the Advanced ENT and Allergy Family. She is married with two children and in her free time she enjoys cooking and watching her kids on the soccer field.

I am originally from St. Louis, Missouri and obtained my Bachelor’s degree in Biochemistry from Westminster College. I graduated from Southern Illinois University Physician Assistant Program with a Master’s degree in Biomedical Sciences. After becoming a nationally certified Physician Assistant, I specialized in Infectious Disease. My family recently relocated to Louisville, Kentucky for my husband’s surgery training and in our free time we enjoy spending time with our daughter, Mia, and exploring Louisville.

Kaela Orwick, APRN

Kaela, began her healthcare journey as a Registered Nurse in the Emergency Department, where she spent eight years mastering acute care and critical thinking. Building upon this foundation, she pursued further education and obtained her Family Nurse Practitioner (FNP) degree from South University in 2022. Before joining Advanced ENT and Allergy, she began her career as a Hospitalist, providing comprehensive medical care to hospitalized patients.

Outside of her clinical practice, Kaela enjoys an active lifestyle and spends quality time with her loved ones. She finds joy in being outdoors, whether it’s hiking or recreational activities with her boyfriend, son, and their dog.

Lauren is a certified Family Nurse Practitioner from Louisville, KY. She graduated from the University of Louisville with her Bachelor’s degree in Nursing in 2017. Following graduation, she worked in the Neonatal ICU for 7 years and completed her Master’s Degree at Bellarmine University in 2023. Lauren enjoys traveling with her husband, going on walks with her golden retriever, and rooting for the Louisville cardinals.

Kendyl is originally from Olympia, Washington. There she earned a Bachelor’s in Nutrition from Central Washington University. She moved to Louisville to attend Bellarmine University and receive a Bachelors in Nursing through their accelerated program. She spent 8 years at Norton Children’s Hospital before earning her Masters in Nursing from Keiser University to become a Board Certified Family Nurse Practitioner. She also has an array of certifications in Aesthetics and Bio-identical hormone replacement.

Kendyl is married and together they have two boys. Kendyl enjoys shopping, traveling, and spending time with her family.

Kasey Fawbush completed her undergraduate studies at Bellarmine University in 2013. She gained valuable experience as a registered nurse at Norton Children’s Hospital and as a clinical manager at Home of the Innocents. Aspiring to advance her career in a more clinical role, Kasey attended Indiana Wesleyan University, where she earned her Master of Science in Nursing and became a board-certified Family Nurse Practitioner. In her spare time, Kasey enjoys reading, traveling, and spending time on the lake with her husband and two children, wakeboarding and wake-surfing.

Sara Odle, BSN, RN, MSN, FNP, IHP

Sara Odle, BSN, RN, MSN, FNP, IHP, is a Nurse Practitioner with over 11 years of experience. She specializes in general ENT, vocal disorders, chronic cough, allergy management, and otology, and has worked with otolaryngology teams. Sara sees patients at our Louisville and New Albany offices. With a holistic approach to wellness, including movement and mindfulness, she is dedicated to improving patient outcomes through integrative health practices.
